Lactic Acidosis is no longer the concern it once was, and the FDA allowed the manufacturer of Glucophage and Met to take the warning off the label.

When metformin is used as labeled, the increased risk of lactic acidosis is either zero or so close to zero that it cannot be factored into ordinary clinical decision making. That metformin can itself cause lactic acidosis is supported by the finding of lactic acidosis in people who took overdoses. Thus, the accumulation of metformin in the setting of renal insufficiency might be expected to precipitate lactic acidosis in some patients who are at risk. If one excludes overdoses, most cases of metformin-associated lactic acidosis, particularly the fatal ones, were probably not caused by metformin.

April 29, 2004 — Last month, the U.S. Food and Drug Administration (FDA) approved revisions to safety labeling for metformin HCl, lisinopril, lisinopril-hydrochlorothiazide, fluticasone propionate, vancomycin injection, and amino acid injection.

Metformin HCl (Glucophage) and Metformin HCl Extended Release (Glucophage XR) Tablets Not Associated With Lactic Acidosis

The FDA approved a revision on March 19 to the warnings section of labeling for metformin HCl (Glucophage), and metformin HCl extended release tablets (Glucophage XR), both of which are made by Bristol-Myers Squibb, to reflect that there were no reports of lactic acidosis during more than 20,000 patient-years of exposure to metformin in clinical trials.
