When we had to do this, my gyno gave us a "prescription"/paperwork to take to the hospital. My husband had to ejaculate via masterbation into a cup and we took the cup up to the hospital. So, he was able to do it from home. As long as you get it to the lab within 35 minutes and can keep it at body temperature (I kept my hands around it) it was fine.
