When I had BCBS of NC through my work they sent out a book that told in plain detail what was covered and what wasn't, I don't think I know of many insurance providers that do that, but it definately made my life a lot easier. If you don't have something like that USUALLY there is a # on your insurance card that you should be able to call and they can tell you what will be covered, what won't be covered, what percentage will be covered on certain procedures and what not.
