Actually, you are right. Some insurance companies make sure that it isn't entirely clear and won't give you a definitive answer when you call, without having the specific procedure codes. That said, your benefits description should at least list what it NOT covered under infertility diagnosis and treatment. If you'd like it post it here, maybe we can make sense of it.
