g. Hair loss - yes, but not sure if it was related to hypothyroidism or PCOS
h. "String of pearl" cysts on ovaries (these are multiple small cysts) - not that I know of (never had an ultrasound to check)

metformin, a little more exercise, dieting (South Beach Diet), Vitex.
