I have a pretty flat tummy, even after having my DD. Yet my cycles have been irregular since I started menstruating and I suspect I may have some degree of IR, too (have not been tested for it). There is type II diabetes on both sides of my family, but no one else with PCOS, that I know of. I am also slightly hypothyroid (on 25mcg of synthroid). Weird.
