Add some protein - yogurt or cottage cheese, maybe string cheese if you can find a low-fat one. Make sure any carb foods are also high fiber. The whole wheat roll may not have alot of fiber in it - check the label, or look for something that says it has bran in it.
By the way, your GYN deserves credit - many don't make the connection between YI and diabetes.
