Actually with your BMI being higher than 40 you pretty much have an open pass for the surgery. Mine was only 39, so then I had to prove all my other issues were weight related, etc. Which you do have as well being pre-diabetic, high cholestrol, & high blood pressure.
Dont worry about not getting approved until it happens. I had Lapband on 12/11 & have lost 40 lbs. It takes work, espically early in the game before you have enough fills for the band to make you feel full. However, for the first time in your life you will something to rely on so you dont give up, as oppose to giving up because your straving to death.
