I had a severe thyroid attack in Jan of this year to, my TSH was 178.84 whoe!!! My Endo put me on 450mcg of synthroid daily to get the hormone down. Once down to 7 she leveled my synthroid to 150mcg but my last test showed my tsh too low at 0.36 (0.4-2.7 is optimal for TTC). Now she has me on 75mcg on Sundays and 150mcg M-Sat.
Largest dose I have heard of is 750mcg. I asked my Endo why some people need a higher dose. She said that everyone metabolizes differently. A woman who is a bit overweight commonly needs more synthroid then women who are thin. But it all differs from person to person. The goal is to level out the TSH and it can take up to 4 mths to do this. She said that running a TSH every month (for those just diagnosed) helps to get the right dose. Once level then a TSH is drawn every 4 months to make that the synthroid dose is working correctly.
Also if you are hypothyroid and TTC or are pregnant then a much higher does is needed in order for both you and the baby to metabolize correctly and prevent miscarriage. I am currently TTC so she has been preparing me for the "when" scenario.
I should clarify that I have Hashimotos. And in the past have had issues with a hyperthyroid. So I am just glad to diagnosed and being treated!
