I don't know how far you are from Toledo, but my beloved endocrinologist is leaving my area to lead the Endocrinology Program at the Medical College of Ohio. He's probably in transition right now, but you should be able to find him there within a few weeks. He will be teaching and running the program half-time, but will also be seeing patients half-time.
His name is Dr. Gregory Tennyson. He diagnosed my PCOS, and I have found him to be up-to-date on the latest research and always willing to listen and give feedback on the research I've done. He believes that all PCOSers are insulin resistant by definition, and puts them all on Glucophage. My starting dose was 1500 mg, but he was not hesitant to move me up to 2000 mg when I asked him about it. I have heard people say that he's not exactly the "warm and fuzzy" type, but we always got along swimmingly, and I found him to be plenty warm enough.
Good luck, and please let me know if you do end up seeing him! I'm thinking of making the 2.5 hour trip to Toledo to see him once a year myself.
