From what I understand breastfeeding uses a lot of calories which would explain the rapid drop in your blood sugar after breastfeeding. You might want to talk to a lactation consultant or nutritionist at your local hospital about breastfeeding while diabetic for some pointers on what you should be eating to keep your sugar levels more even, so you don't experience the highs and lows. Even if you are IR and not diabetic it could help you.
The elevated liver enzymes could be unrelated or from the glucophage before you were pregnant. A lot of medications including glucophage are filtered through the liver and can cause damage in some people.
However, my little sister has 3 children and she is not IR or diabetic or on any ongoing medication and she now has severely elevated liver enzymes. She has what they call a fatty liver, also she mentioned NASH (but I can't remember what the acronym stands for). The doctors say it is most likely from having her three children so closely together; they added too much stress on her system.
The treatment for her is to avoid fried foods and fatty foods and basically eat a low cholesteral diet.
Best Wishes,
