A few weeks ago I had a school assignment which necessitated having people complete surveys about their cultures. Now I have one about spirituality. I'm
interested in hearing about lots of different religions and methods of spirituality.

Here are the questions:
1) Do you consider yourself spiritual or religious? Why or why not?
2) Do you belong to an organized religion. If so, which one?
3) How important is this religion to you and to your family?
4) What parts of your religion do you practice regularly (i.e. attending mass once a month, going to prayer circles each Friday, etc.)
5) If you are in a hospital, would you like your religious group contacted?
6) Has there been a time when illness has interfered with your religious practices? If so, how?
7) Are there any symbols, books, or other things that would be helpful for your practice when you are in a healthcare facility?
8) What are some ways that nurses can assist you in your practice during a hospital stay?
9) Are there parts of your religious life that are affecting your health (fasting, food restrictions, etc.)?
10) Describe what gives meaning and purpose or brings joy and peace in your life. List activities that help you accomplish this.
11) What do you do during times of stress or challenge to find comfort?
12) List three ways you achieve spiritual balance in your life.
