It was taken off the market for a bit due to manufacturing issues and it is not back on entirely... at least it wasn't a month or so ago. I was taking it at the 2/1000 level for a bit but then it got pulled from the market so had to go back to taking (and paying for, ugh) each med seperately. For some reason they have it back at other dosages but not the one I need. I guess they're working on it.
I don't know how your doc decides to put you on it and not just one or the other. Personally I was on met forever and it wasn't working entirely - helped with weight but that was it - so my doc wanted me to try avandia, too - avandamet made sense due to ease/cost. Seems MOST women do met alone first and then add avandia if needed... there are cases of docs thinking avandia is best to start and then maybe adding met later... but I am not sure in what situation a doc would immediately jump to giving both.

We never talk of our lacking relationships
And how we're guilt-stricken, sobbing, with our heads on the floor
We fell through the ice when we tried not to slip..."
