Could this be a PCOS issue? I use deoderant/antiperspirant regularly but for some reason my right armpit still get BO. It is soooo frustrating!

I tend to get that with my left armpit. Not sure why. I tend to sleep on my left side more. who knows

BO is caused by bacteria on the skin. Try using an antibacterial soap like dial on your armpits. I wouldn't use it on the whole body though cuz it tends to be drying to the skin.
