They did an US on me today and I have 3 fibroid tumors(funny I didn't get these till I was put on the pill) and they did US internally and externally and I don't have any cysts on the ovaries at all.
I have PCOS and no one has ever found a single cyst on my ovaries. I am new to pcos so I can't tell you much about it...just wanted to say that. take care.

Oddly enough, you have PCOS without ovarian cysts AND ovarian cysts without PCOS. That's only one reason why this disorder is so hard to diagnose. There are several symptom patterns that can show up and it seems like no one can agree on exactly what PCOS is.
My gynos were always totally clueless. I ended up being diagnosed by an endocrinologist. The first doctor who mentioned it to me was a family practitioner, though.
I totally agree with you girlygirl522 - i was given the run around for so long by my doc that I finally took charge - put my foot down and demanded an answer or a referral. i got the courage from a few of my coworkers who were having to put with me and my symptoms and pain, and whatnot. I was at my wits end until I saw my endo. sad thing - she left her practice to move out of state, so i am an endo short right now. but my ob/gyn is up to date on pcos stuff and we are in the middle of seeing if i need clomid - hopefully we will get on the right track - if only my ovaries would do what they need to do. my doc says she doesn't see the point in doing an us this early, i may have on down the road, even though we have been ttc for over a year now, and am just trying to get a period at all. I don't know if i have any cysts, but i do have all the other symptoms of pcos.

I haven't had an ultrasound to see if I have cysts or not, but I think I have them because I'm pretty sure that one ruptured last month. Other than that I wouldn't have had any idea if I had them. Neither of the gynocologists I saw when I was being diagnosed felt it necessary to do an ultrasound to make the diagnosis. Usually it's based more on hormone levels and symptoms.
It is possible that you had them before the ultrasound and then later after the ultrasound. When I had my first ultrasound I was told the same thing - no cysts, then a couple of years later when I had an MRI of my left hip, lo and behold, cysts on my ovary! When I asked my doc about this she said they come and go, and not to worry about what was seen at the time.
This is exactly the issue that led me to be misdiagnosed by my last GYN. During laparoscopic surgery to diagnose and remove Endometriosis he proudly crowed, and even took pictures, of how "BIG and healthy" my ovaries were. He saw "no signs of cystic activity". PCOS was then removed from my chart despite being diagnosed by no less than three other GYNs at various times in my life. I was diagnosed by symptoms (of which I have all but the cysts) and extensive, repeated bloodwork. All of that bloodwork came back screaming "classic PCOS" in the words of two of my GYNs. I also later learned that enlarged ovaries is often a sign of PCOS, and even that idiot of a doctor admitted that they were quite enlarged.
I agree with what many others have said; see a Reproductive Endocrinologist if you can. They will be able to fully figure out what is going on and are more up to date on knowing that ovarian cysts are not necessary for a PCOS diagnosis.

I have PCOS and my ovaries are fine. In fact, at my last ultrasound, the tech said that my uterus and ovaries look "textbook perfect". She said she wouldn't have known I had PCOS from looking at my ovaries because I have no cysts. My ovaries did look a bit large, but that was during my second Clomid cycle and the tech said that it was normal.
