I could be wrong here but it is possible you have two eggs ready to release at the same time. That is what causes fraternal twins, a malfunction in ovulation. Normally one ovary creates a mature follicle and the next month the opposite one does. In some cases, a mature follicle either develops on both ovaries and are both released during the cycle or one ovary makes two mature follicles and releases them both. I would imagine that if you have two follicles ready, they could both release at different times or one will release and the other will not. I would question if you don't have some malfunction in the Luteinizing hormone that would cause that.
