If possible, can someone please explain to me how the cervix works. I was 2+cm dilated, 1.5cm cervix left, bulging membranes, twins, 80% effaced. When my doc tried to do a rescue cerclage, he was only able to fit 1 stitch and it ended up not working out. However, someone else who was 7cm dilated with with twins, bulging membranes, was able to fit 4 stitches, which are working. So I guess my question is....when it comes to how many stitches can fit, does it depend on the cervical length, width, or both? The 1.5 that I mentioned above means the length..how LONG it was (vertically)...right?? And the 80% effaced means how thick my walls were and how much width I had (horizontally), right?? So what determines how many stitches they can get in? Do you know what I mean?

My problem is that I need to know the medical aspect of things...but I'm not a doctor. I know, I know...ask a doctor. Unfortunately, I spent my time with the doctors yelling at them for making me feel horrible, instead of asking the medical questions like I should have been doing. I guess I'm now at the "why her and not me" phase.
My short answer is that each case is very different, and the skill level of each doctor is very different. Of course we don't usually know ahead of time that we need the best doc for this project! I think talking to some doctors who have performed successful rescue cerclages would give you your answer. Each one probably has their own techniques for getting all of the cervical tissue together to work with. And then they would be fighting against contractions with most patients, of course. Sometimes those just won't stop, so it becomes more difficult.
I just know that with most of our cases, we are indeed able to carry to term with proper care. We simply need the right people working for us and watching our details, and we can do it. The ones who tell us, "You're not a typical IC case, etc." and so don't watch us or do anything preventative are the doctors I want to strangle. Don't ever listen to that crap. Move right on to another choice. Prevention is better than rescue!!! The constant reassurance of u/s cervical measurement keeps our spirits up during those crucial second trimester weeks and lets us know if we need more rest, etc. It's all for our babies and very worthwhile.

Well, that explains it somewhat. I wasn't sent to a specialist until my "rescue" needed to be "rescued" and by that time I was leaking fluid and they wouldn't do it because the risk of infection was too high.
I requested a copy of the report from my cerclage...he said he put one stitch in but couldnt "really" fit in another one so the procedure was terminated. He probably just didn't know HOW to fit another one in. He's just a regular OB/GYN.
Also, I was sent home on bedrest with no contraction meds. When I thought I was having contractions, I called. I told them I think I'm having contractions but that they seem to be ending in gassy bubbles and they told me it's probably just gas. Also, I seemed to get them only after taking my antibiotic (Zithromax) so they just told me to stop taking it and see what happens. But I never got to see if that helped, because the next day I was checked and found to be dilating again.
I still don't understand why regular OBs think they can handle surgery! Catching a baby is a lot different than trying to keep one in.
After my rescue cerclage, I was sent home with a two-week supply of Indocin. I took my last one, and I went into labor (PROM) that night. I don't know if the infection was already in there, or if there was some other drug I could have stayed on.
At least next time I've got decent docs on my side, and I'm praying for the same for you all!
