Could this be PCOS? Hi- I am feeling so confused after seeing my RE. 8-ish years ago I was "diagnosed" with mild PCOS, because of hirtuism. No bloodwork was ever done, and shortly after that initial visit I got pregnant with my first child. A couple years later it was taking a little longer (and by that I mean like 6 months) to get pregnant again. I visited a new OBGYN, who told me, "Yes, you have PCOS- I don't need labwork, etc". She put me on metformin and I was pregnant that same cycle with my second child. She had me stop taking it when I got pregnant, I then breastfed for 2 years so didn't go back on. So, last July, after trying 15 months to get pregnant again I saw a new OBGYN (we have moved around a lot) and she said the other docs were right, I have PCOS, and gave me metformin since it worked so quickly last time. I got pregnant in August but unfortunately miscarried in October. At that time the doc switched me from regular met to extended release since the regular was causing so many intestional problems, but it didn't have that same magic effect because I have not been able to get pregnant again. We will have been TTC #3 for 2 years next month.
So last month I started seeing an RE. She told me that she didn't think I had PCOS at all, just excess androgens. My cycles have always been regular, she checked my ovaries and they were normal with no cysts. She did a ton of bloodwork and told me to stop taking the Met. because she didn't think I needed it, and that was why it wasn't working. After being off it for a month she wants a 2 hr glucose tolerance test and fasting insulin levels, which I will do this week.
But after my inital bloodwork she still doesn't think I have PCOS. My LH:FSH ratio is 1:1, my antimullerian hormone is 1, she said in PCOS patients it is above 4. All of my "ovarian" hormone tests came back normal.
Here is what didn't come back normal:
Testosterone was 60- she said they like it less than 40.
DHEAS was slightly elevated although she never gave me the exact number
17OHP was 250 and they like it 200 or less (this was taken the day before my period, they are rechecking it when I have my other fasting labs done since it will be early in my cycle)
My cycles have always been regular. Doesn't this sound like PCOS?
We are moving on to IUI this month, and I really don't want it to be a wasted effort if there is no way I can get pregnant because of those hormones being elevated. She actually diagnosed us with "Unexplained infertility". In all liklihood these hormones have been elevated for a long time, and I didn't have much trouble getting pregnant the first 2 times. It sounds silly but I feel really weird about not taking the metformin- of course I don't want to take a drug if I don't need it, but I'm just having trouble believing I don't need it. I am slightly overweight, but not obese.
