My parents fostered and when one of our foster kids was adopted by another family (she was 6) they changed her name. My mom ran into her now that she's an adult and she doesn't even remember being her previous name except for living with us. Mom said it didn't seem to have any negative impact on her.

We adopted our daughter at 3.5 years of age and didn't change her name. She has a lovely name and there was no point in changing it and confusing her.
