The suitability of laparoscopic excision as first line treatment for endometriotic ovarian cysts has recently been questioned for fear that it may damage ovarian reserve, a concern that Italian researchers have now demonstrated is real, but small.
Mauro Busacca (Ospedale Macedonio Melloni) and colleagues studied 126 patients who had undergone bilateral excision of ovarian endometriomas and found that three (2.4 percent) experienced ovarian failure immediately after surgery.
Commenting on the significance of the research in a related editorial, Harry Reich (International Society for Gynecologic Endoscopy) and Mauricio Abrao (University of Sao Paulo, Brazil) said: "Without doubt, the importance of this article is that it concisely discusses the possible causes involved in this situation, including: irreversible trauma to ovarian vascularization caused by electrocoagulation, excessive removal of ovarian tissue, and an autoimmune reaction caused by a severe, local inflammatory process."
