Endometriosis severity affects IVF outcome

Evaluating the impact of moderate-to-severe endometriosis on cumulative IVF outcome, compared with that of less severe disease.
Women with moderate-to-severe endometriosis are less likely to fall pregnant and have a baby with the help of IVF than those with minimal-to-mild disease, researchers have shown.
"Women with endometriosis often need IVF to conceive—most women need several cycles of treatment," note the researchers, led by Paula Kuivasaari from Kuopio University Hospital in Finland.
To investigate the impact of endometriosis severity on IVF outcome, they followed-up 31 women with stage I/II (minimal-to-mild) endometriosis and 67 with stage III/IV (moderate-to-severe) disease who underwent IVF or ICSI, along with a reference group of 87 women who had tubal infertility.
Pregnancy rate per embryo transfer was significantly lower for women with stage III/IV endometriosis than for those with stage I/II disease or for the controls (22.6 percent vs 40.0 percent and 36.6 percent, respectively). However, after 1-4 cycles, 56.7 percent of women in the stage III/IV group were pregnant and 40.3 percent had given birth, compared with 67.7 percent and 55.8 percent of those in the stage I/II group, respectively, and 81.6 percent and 43.7 percent of controls.
The authors say it is "obvious" from their data that the success of IVF decreased with increasingly severe endometriosis, but also point out that "the overall cumulative outcome of IVF/ICSI treatment was favorable."
They add: "Pregnancies in women with endometriosis more often resulted in live-born infants than those in the tubal factor group."

Yay!! So happy for you.
I am off to work on a paper for my classes. Hubby says we will work something out so everything will be fine. We want me to at least finish my master's and I have to stay at the job I am at to do that b/c I am in a state program. If we HAVE to put the IUI on hold we will I guess. |

I have been feeling pretty crampy since I have been taking 100 mg of clomid...last time was only 50mg. My legs and stomach are cramping pretty bad. I do find that it helps to keep going and move around. I get a lot of cleaning done this way at least.
Leah-I feel so bad for you, but I get that way sometimes too. Take it one day at a time. It will all work out, although it hard to see that sometimes from where you are standing now. The IUI is not an invasive procedure and it doesnt take that long either. You can go to work the same day....maybe 2 hours at the most you would miss. You will know when the time is right for you to do it! Hang in there and know that we love you and you arent alone! We can go through this together. We hopefully are doing the IUI this month, so I can give you the details...I have to ovulate though, so its no simple task! Let me know how you are soon so I dont worry! Please dont be so hard on yourself.

Thank you so much. I needed that. The problem with the IUI is that my doctor wants me to come in every other day while on the injections and do u/s to monitor and make sure I am not overstimulating and such. So it would be about a month of going to the doctor every two to three days. That is why hubby had said if possible and need be I could take medical leave without pay. But they are going to consider that a personal need, not medical. But I am doing better and it will all work out. I am letting God take over and having faith. As well as trusting that me and hubby will figure out the best thing. I can't leave this job due to the master's program I am in b/c I want to complete that, which will be another year. If we have to postpone we will but we don't want to. I will have you in my thoughts and prayers and ************STICKY DUST***************** to you. We had actually figured out the money issue with the IUI. So it will work out.

So I'm exhausted today since I was up last night with J for the most part talking -- His ex is a complete mooke beyond belief... Everything that Karl did to me Last monday with the proposal the guilt trip ect.. She did to him last night. She even pulled out the crying card, "What I didn't make you happy" card, "What do you like about her more then you do me" card... Its very frustrating really She even at the end of the convo pulls out the "I hope I hit a tree and die on my way home tonight" card. OK WTF You had a year to try and get him back and as soon as he's happy BOOM "I need him" BS... I don't get it and he's such a big softie and he worries about people so of course he was very upset. She sat there and tried getting him back then got defensive and started putting him down. Someone that loves someone else so much and wants to spend the rest of there life with them DOES NOT DO THIS... Seriously If she pulls this crap again someone will have to come up and bail me out of Jail because her and I *WILL* have words. Its just not fair. I tried Joking around saying maybe we should hook up our exes LOL He chuckled then went back to being upset. Then I suppose she told him that No they can't be friends that she never wants to talk to him or see him again blah whatever.. Personally YAY for me Great stay away because I knew this was gonna happen... *sigh* he stayed strong and kepts saying "I am in love with L, She makes me happy"
After he finally got calmed down, eyes watery He looks at me and he's like "You love me, I can tell in your eyes" I told him yeah probably more than anyone else before... Then He asked me never to leave him (unless things got REALLY bad) because his heart couldn't handle loosing the most important thing in his life... A few mins later looks at me again... "I'm going to ask you to marry me and I hope you say yes" I was like well when you ask you will get an answer but we'll cross that bridge when we come to it... He's like oh well we are going to cross that bridge.... He then Kinda Chuckled and was like "Your gonna marry me" with this smile on his face and I said yeah someday I will --
So thats my crap of the day ... well some good some bad...

It's his past, and since he loves you I'm sure he'll get her far away. I had a friend once that used the suicide crap to get the guy to care for her, she would even down pills right in front of him, digusting. I remember one time she downed a bunch of pills and called my ex (who was a paramedic). He shows up and she's in a nightie with nothing on underneath. She was very jealous that I had a man and she didn't, so she had to do something. Needless to say, she's not my friend anymore! I am so glad that things are going well with J

Investigating whether current and historical body mass index are associated with a diagnosis of endometriosis.
Women with endometriosis are consistently leaner during adolescence and early adulthood than other women undergoing laparoscopy, research shows.
The investigators say the findings support an early origin of endometriosis in utero or early childhood.
Mary Hediger (National Institutes of Health, Bethesda, Maryland, USA) and colleagues assessed the association of current and historical body size with a diagnosis of endometriosis on laparoscopy.
In all, 84 women aged 18-40 years, who were undergoing laparoscopy for tubal sterilization or as a diagnostic procedure were included in the study. The women were asked to recall their current body mass index and their perceived figure for 5-year intervals starting at age 15-19 years up to their current age.
After adjusting for age, being tall, and parity, the researchers found that a higher current body mass index—whether determined by self-report or perception of body figure—conferred significant protection against a diagnosis of endometriosis.
In an adjusted repeated measures model, which was used to look for longitudinal trends, BMI was 21.3 kg/m2 for cases compared with 23.2 kg/m2 for controls. Assuming an average height of 64.5 cm, this equates to a consistent difference of about 10 pounds at every age, the researchers explain.
"We do not anticipate that a lean physique will be a sensitive indicator of disease; however, it may provide an additional piece of information helpful in developing a clinical profile of susceptible women," they conclude.
