So sorry this happened to you. Fainting can be very scary!
You seem to be having hypoglycemic (low blood sugar) episodes. Sometimes, this can be a precursor to diabetes, so it's good that you're going to have a fasting glucose test done. Hopefully, the doc will also do a fasting insulin test and a glucose tolerance test in order to thoroughly evaluate you. Yes, diabetes can be a consequence of having PCOS, but you may be able to catch any blood sugar/insulin problems early before it actually progresses that far.
Something else I hope your doc tests you for is anemia. Anemia can also contribute to the dizziness, and since you are passing big clots you are at a higher risk for it. I have heavy bleeding problems due to a very large uterine fibroid, and I have been on prescription iron supplements for some time now to help battle anemia.
Hope you can get this all sorted out quickly.
