I type medical records for a living and doctors, nurse practitioners, etc., say that all the time. It just means you know who you are, you know where you are, and you know when it is. Sometimes they say, "alert and oriented times three." It just means that you're normal. If you thought it was 1973, or if you weren't sure where you were, or who you were...that's what they mean, real extreme stuff. Grossly just means obviously, as in she probably didn't ask you to tell her what year it was because she figured you were fine just by talking to you.
So, it just means you're all there!
