It's hard to say what this could be since we don't know your medical history, weight, meds etc. It could simply be swollen lymph tissue from inflammation or an infection, it could be simple fatty tissue lumps (as a woman with large breasts I'm very familiar with these), it could be cysts or fatty tumors. My husband had a fatty tumor removed from his shoulder last year. They're generally non-cancerous and it was easily removed.
What do the doctors say about the lumps? Can they feel them? Are they visible from the surface? Are they painful? How big are they? Do they change right before your period? Have you had an ultrasound of the lumps?
As for fatigue it could be a million things from lifestyle change to anemia to sleep disorders. Without more info there's no way to tell.
