Your free T4 is in the lower quadrant of the lab range. Optimal would be midpoint or in the upper quadrant. Your TSH is highish (3 is now the upper cutoff, but many labs still use the old range that yours did). If you are having symptoms, then I would pursue getting a free T3 and a TPO antibodies test (to check for Hashimoto's Thyroiditis, a common condition among women with PCOS).
Many docs wouldn't diagnose or treat you with those numbers you have. Others would do more tests to rule out other things (ferritin, female hormones, cortisol, pituitary, etc) and put the pieces of the puzzle together, including your symptoms, before deciding whether to start you on treatment. Sometimes the available tests just aren't sensitive enough to tell if one is hypothyroid or not, and a doc might prescribe meds to see if the symptoms improve (if other conditions are ruled out).
