Diagnosable Insulin Resistance occurs with PCOS in about 40% of the diagnosed PCOS cases. In essence, someone who is insulin resistant is on the total opposite end of the spectrum than someone who is diabetic. In a diabetic, they either do not produce enough insulin on their own or none at all. For someone who is IR, they produce WAAAAYYY too much. The effect of all this extra insulin floating around in your system is much like taking too many antibiotics...over time, your body builds up a resistance to all this extra insulin, and ignores it.
What happens, in layman's terms, is this. You eat a meal, and your insulin is released to carry the glucose to your cells. Your cells have become resistant to your insulin, so it has no where to deliver the glucose. After a period of time, your brain senses that your sugar level is still too high, and orders more insulin to be released to bring it down. Thus begins a nasty cycle. For someone who is insulin resistant, it becomes gradually worse as time passes, and it gets very easy to gain, but very hard to lose weight.
It's a catch 22 situation...as your weight increases, so does your insulin resistance. Luckily, the converse is also true. As you lose weight, your insulin resistance also decreases.
This is where metformin can be helpful. What metformin does is to resensitze your cells to your own insulin, so that you can use your insulin more efficiently. The drug itself is NOT a weight loss drug. All it will do is "level the playing field" so that if you follow a healthy diet and exercise, you can lose weight like normal people.
Now, the side benefits. Insulin is a master hormone. When one of your hormones gets out of whack, the other hormone levels in your body move up or down to compensate. This changing of the levels can cause all kinds of problems in a woman's body...hair loss/growth, reproductive problems, skin problems, and a general feeling of fatigue and even water retention. For many women, whether they are overweight or not, met helps to get these hormones back into balance by working on your insulin. Often times, taking Birth control pills or other medications that work on your hormones can sometimes throw a monkey wrench into the works...they may mask or "undo"the work that the met is doing. For others, their docs recommend it, and they do not seem to have problems. This is where communication with your doctor is vital.
Don't assume that because you are not losing a lot of weight that the met isn't working. For some of us, it takes more time to get the hormone levels where they should be. Be patient with yourself...sometimes, it may take a dosage adjustment to find what's right for you.
For those who are not dxd IR, doctors are also using met to help cysters who are having trouble ovulating or other reproductive problems. There are many cases right here on this forum that met has helped bring on AF, regulate cycles, and even helped some to conceive, even after trying clomid. The underlying key to all this is the hormones....they must be tested, and re-checked to be sure the met is helping, and the levels are moving in the right direction.
Your way of eating is important to your success with met. Even if you are not actively trying to lose weight, your results with met will be enhanced by eating a lower to moderate carb diet and exercise. Think about it....met is a Type II diabetes drug. Obviously, for a diabetic, they would make things harder for themselves if they did not watch their diet in order to feel their best. Since this really is a diabetic medication, the drug will work its best when your body is not flooded with carbs. Don't make the medication work harder than it has to, and we cannot expect to sit back and let the drug do all the work. Now, I do not mean to make taking met sound like roses and wine. For some, they simply cannot tolerate the drug, and for some, they do not experience the results that others do. As far as allowing the drug time to see results, trust me...it will take more than a couple weeks to really see what's going on.
Hope this answers some questions about met...we are all here to lend support to each other and share information. The best thing you can do to get your health back is to read everything you can get your hands on. Read the research articles on this site...learn all you can... there is new research coming out all the time, and as some of us know, we can end up teaching our docs sometimes!

The article at the beginning of this thread explained it better than I have ever heard it before, so thanks! I was prescribed with 2550 mg of Met, after starting with 1000 a day. However, I have not been very good. I had stopped taking the Met. I was really kind of ignoring the whole PCOS thing, getting more and more frustrated, b/c I would eat normal meals, sometimes not eating very much b/c I just wasn't hungry, but still gaining, never losing no matter what kind of exercising or how much I was doing. I recently decided to get serious about treating the PCOS, and starting taking the Met again and enrolled in WW Online. I stumbled onto this site, and started reading the threads, and for the first time since my diagnosis, I felt good about not being in this battle alone. It also did me good to know my Met side effects happened to lots of people. I have learned some things about the Met as well. This time around, I want to make sure that I keep up with the Met, b/c going back on after being off for a time is bad as far as side effects go. I am taking it slow, working back up to my 2550 a day (does anyone else take that much?), and look forward to seeing results now that I have a better understanding of PCOS and Met and IR.
On another note, I also have RLS, bursitis, and allergies, and take additional medication for each of these. All of these diagnoses have come about around the same time, so I went from absolutely no medication to 5 different medications, all in less than 2 years. I have found that I cannot take Met at the same time as my RLS medication, and if I take Met past 6 or 7 pm, I feel horrible all night. How do others deal with multiple medications in addition to the Met? Do others have some of these issues? |

I notice that a lot of ladies who have managed to lose a lot of weight on met and diet/Exercise are still taking large doses of met. I take 1500mg a day now because after I lost 45lbs it made me very dizzy and nauseated to take 2000mg. My Doctor says this is because my IR is improving and I needed to lower my dose. I am assuming from the amounts still taken by you ladies that this is not the case?
I was not sick on 2000mg when I weighed 225lbs all the way down to like 190 but then I started to get very very dizzy and nauseated, dropped to 1500 and stopped feeling sick.

Thanks for posting this info. I knew what IR is because my dr drew me a picture with the cells and glucose and insulin and other stuff. So it made it really easy to understand. But your post had good info in it.
Im also following the IR diet. I don't cut out carbs entirely, just eat better/healthier whole wheat carbs, and more protein. But if I want spaghetti, I eat it. Just smaller portions.
So far (been taking 3 weeks) my AF hasn't returned on its own, but at first the acne was clearing up. I thought my whiskers weren't growing in as fast either.
Greasy food and high carb food makes my tummy icky faster. But I realize that each person is different, as iceberg lettuce doesn't upset my tummy.
I've lost prob 10 to 15 lbs since getting the meds. Hopefully this continues 
It would be awesome to give my DH a BFP this year

Points: 1,073.58 Bank: 1,038.06 Total Points: 2,111.64 | After reading so many posts on the board about Met I think I'm going to call my gyn to see if I can get a prescription. He offered me this or BCP and I asked him to give me whichever will give me more energy -- so got the BCP. I've been on Atkins for four months and have only lost 20 pounds (while other people who weigh less than me and don't have pcos have already lost 40). I exercise and take the recommended supplements so the next step is to try the Met to see if that will boost my insulin sensitivity and help with the weight loss. Thank you everyone for sharing part of your lives here.

I have had PCOS for years and I'm hopeful that I am on the right track to effective weight loss. I'm glad I found this web site as a support system!
p.s. I tried atkins, too, and only lost 5 pounds after weeks with no carbs!! I hated it! I have an awesome cook book called "The Good Carbohydrate Cookbook" by Sandra Woodruff. It is great and even has a section on PCOS. |

I'm hoping to find the key to working with PCOS here too. I keep thinking that there must be something that will help with the weight loss. I do get frustrated at times, but I hang on because I don't want to get diabetes and everything else that goes along with PCOS. So, I continue to eat this way and if I lose weight, then it is a bonus. Thanks for posting and I look forward to seeing what your dr has to say about your bloodwork.
