Best wishes for a positive outcome! I have a strange situation. I am late 30's plus size women who just was Dx with PCOS.
I have a great RE/GYN/OB who has tried to do both a SIS and HSG test. He was not able to compete either test. He chalked it up to my anatomy and being to large. He said that he couldn't get to my cervix... (Sorry for TMI!)
He gave me two options - have gastric bypass surgery to lose weight quickly and then have IUI or just try Metformin and Clomid and try to get pregnant with those. He said he would not be able to do IUI unless I lost a lot of weight quickly or he found a bigger instrument... GOSH this is really too much info - sorry ladies!
I was just wondering if anyone else has had this problem or been told this...

I haven't had that situation - but I just wanted to say good luck to you!!
Sounds like getting on Metformin should be your first step. It can help you in so many ways including weight control. I definitely think getting healthy may be the first step!
The GB is a huge step, is it something you thought about, or want?

Thanks for the reply. I have started Metformin, will be started Marvelon and Clomid later in my cycle. The Metformin scares me a little, because of the side effects... But I will do anything I need to do to have a baby.
I said no thanks to the Gastric Bypass.... That just doesn't appeal to me at all. I am working with a Natropath who has a lot of great advice. She says that I just need to get in better health and the baby thing will fall into place.

I take Metformin - I am not Insulin resistant (so it didn't cause any weight loss), but I didn't have weight to lose, so the doctor said that was normal. I was eased onto it, were you? I take 1500mg per day, I started at 500 for a week or as long as it took me to tolerate it, then 2x then 3x. I have no S/E anymore...it takes a little while to get used to it. And if you go off for a little while it also takes a little while.
My guess is the Metformin WILL HELP YOU, and if you are IR or at risk for Gest. Diabetes it will TOTALLY Help, aslo reduces M/C rate! I will stay on it through my first tri. If you are IR they sometimes will keep you on the whole pg.

I've been heavier much of my life. I've been on metformin off and on for 7 yrs.
For me, I would rather deal with the side effects of metformin, than allow my health to decline.
Also, once your on the highest dosage of Metformin for 6-8 weeks, the s/e dramatically decrease.
I don't know if you've found it, but there is an entire forum on this site specifically for Metformin.
I also agree with Niki, naturopathic medicine is a great resource for PCOS.
Oh, I forgot to add, that I've lost 70lbs from Feb.-July this year, taking Metformin, and following a pretty healthy eating plan, that is lower in sugar/refined carbs/starches.

Samie - Congrats on your weight loss! This is a perfect question for you - what did you do about Met and PG. I am not IR, but my RE is of the opinion he doesn't want to change anything since the Met can't hurt. He said when I do get PG I could stick with the Met through T1 then stop, after T2 starts. Just curious what the normal procedure is with Met. and pg? I shall check out the Met Forum as well!
I personally don't really have ANY side effects on Metformin once I am used to it. I had to stop 2x's for 2 surgeries and I had slight S/E coming off and going back on, but once it's steady for me at the 1500 I feel great!
I agree that 6-8 weeks after you shouldn't notice it.
For me adjusting to the Met I tried to eat frequent small meals and keep a Coke (not diet) and an Apple Juice or Apple or such on hand. I found I'd get light headed and some sugar helped if I got up and felt dizzy....didn't last long for me, and I think About 2 weeks after full dosage I was fine!!

I personally will take Metformin through my entire pregnancy next time, and postpartum as well. I've discussed it w/ my Ob/Gyn, and we fill that any of the risks, are limited, and the benefits will outweigh.
Now, I have a different take on Metformin and the whole IR thing. I personally feel that if women respond to metformin and it helps them, then they actually ARE IR. Because insulin resistancy occurs at the cellular level. It does NOT always show up in blood tests, even the "fasting insulin" one.
The other things to consider is about 40% of women who have PCOS also have supply issues with breastfeeding, because during puberty even if you weren't dx PCOS then, it was already interfering with your breast tissue development. Studies are starting to show that when women w/ PCOS or IR take an insulin sensitizer during their entire pregnancy, and post partum, it dramatically helps, because it allows the body to help kick start breast tissue development.
So if you are planning to breastfeed. I would STRONGLY suggest you look into staying on the Met. If you don't stay on metformin, look into Goat's rue, specifically.

hey super sonic i am heavy as well. although i sis not have any issues with my HSG i was told by many obgyn's to loose weight before even attempting the thought of getting preggers. i started going to the infertility department after i found out i had PCOS, they never ever demanded me to loose weight. they do say that i need to be healthy. i focus on trying to be healthy not the # on the scale. i started clomid and have lost 40 lbs so far... much more to go but its a great start w/ just metformin alone!!! GL to you gastric bypass never appealed to me either.

Supersonic - I am plus sized as well (5'6" 290 lbs). I never actually ended up trying IUI but we had discussed it as our next option if the clomid failed. The fertility specialist never told me that I needed to lose weight to try it. Nor did I have to lose weight to conceive on clomid.
Although it is better for your health to lose weight, I don't think that it should prevent you from attempting whichever method you wish to choose.
Also, I don't think that your weight would prevent the doctor from being able to reach your cervix. The extra weight is usually on the outside, not on the inside. You may have an extra-long vaginal canal making it difficult to reach the cervix, which is the case for some women, but it does not have anything to do with weight. And it certainly would not be something that could be fixed with gastric bypass surgery.
From my experience I have noticed that a lot of it has to do with the doctor. There are a lot of doctors out there that do not like overweight women very much and some that will not even try to help us out because our cases can prove to be more difficult.
I know you mentioned that you have a great doctor but I think that it wouldn't hurt to get a second opinion, just to be sure.

What the hell?!?!? Doctor's now days. Ok mine could not get to my cervix either for 2 reasons. One becuase it's WAY back there and the other because it was so small the lil tube could not fit!! But the very next cycle they gave me something to dilate my cervix (not fun at all but it had to be done) and then they propped my behind up to see properly I think your doctor was being lazy becasue if he wanted he could have found your cervix dang nab it!!!
I agree you need to lose weigth to have a healthy baby I agree with that but at the same time there are women out there that are overweight and are extremely healthly!!!! I AM ONE of them! When I started 3 years ago at TTC I went to the same RE I am seeing now and I weighed a whopping 394lbs!!!!!!!!! She still wanted to help me then something told me that I had to lose some weight I opted for Lap band surgery got it lost 102lbs went back to her just 5 months ago and have gotten pregnant twice which resulted in 2 miscarriages. Then thing is losing the weight does help ALOT, and staying on met I feel helped ME as well to get pregnant. Oh my present weight is 288 (5'8) and I got pregnant at that weight so where it is possible you do need to have a doctor that is willing to work with you not aganist you. I say look for another RE or put your foot down to him!!! GOOD LUCK SWEETIE!!!!

I am plus sized and my RE said he would like me to try to lose weight but he has made my weight an issue. When he did the HSG they had a big of trouble finding my cervix but that was because i was tensed up and once i just relaxed my whole body he had no problem. I've been on met for almost 4 months and have not lost any weight though.

Thanks Everyone!!!! I feel so much better since I found this forum!!! Your advice has meant a lot!!!!! I am going to start the MET, clomid and Marvelon and see how it goes. I think I just need to do one thing at a time and not stress about this. My husband is having a hard time with all of this... We just need to relax I think! He is such a great guy!!!
