You live quite far away from Mayo.... were you aloud to do your ultrasounds and blood work at another hospital and have the results sent to mayo, or did you have to make all the drives.

Haven't heard of Dr. Daftry but the endocrinology and reproductive endocrinology departments are separate. The RE dept is over with the obgyn section (or it was) so brace yourself to see babies while you wait. I know for some women it can be a touchy situation to go in for infertility stuff and be surrounded by pregnant women and babies in the waiting room.
They will let you do labs in another facility and have them faxed/sent in - it's a bit of a pain to do that sometimes as you have to get the labs done ahead of time and check to make sure they get sent etc etc. so I've never really done this. At one point I lived in Rochester so it wasn't a big deal, we moved over to one section of Iowa but I still commuted to Rochester for work once or twice a month so I just kept going to the same doc since there wasn't one in the part of IA we moved to. Same thing when we moved to North of the Cities - no Endo or RE there so I would have had to have driven down into the cities to go to UMN or one of the other clinics to see an endocrinologist anyhow - and I was still commuting to Rochester once a month for work - so I just kept going to my doc at Mayo. I did ask about getting labs done elsewhere and my Endo was willing to let me do that, so I would guess they would let you too *fingers crossed*.
In terms of Zach - I did a harsh/strict program of diet and exercise for 3 years leading up to the pgcy, ended up needing metformin because my insulin resistance was still so strong (even after losing almost 40 lbs and going from size 22 to 14). I went off of birth control and we tried for exactly a year. Since I already had my PCOS diagnosis I knew it could take a while - and didn't want to wait while time ticked by either so at the year point we started the ball rolling to do more - we were about to start a round of clomid (and then IUI and possibly IVF) when I got pregnant. I had just seen one doc over on the reproductive endo side (sorry can't remember his name!). It felt like an eternity to me (because I'd been preparing for 3 years at that point) but I suppose for some PCOS gals a year of active TTCing is not really that long.
Anyhow back to you - I've seen quite a few docs at mayo for various issues - most of them seem like good docs - so I hope you get a good one too! I would bring up your concerns about the clomid at your appointment. They may still want to do a few rounds of it, but I think if you have your past history with you (where clomid hasn't worked) they might be willing to jump straight to injectibles.
