Question midcycle tissue, long period, midcycle pain

Hi. Here's my background info: I am 30 and was diagnosed with PCOS 4 years ago. I am not on BCP b/c they cause my blod pressure to go too high and I'm not on metformin b/c my one insulin resistance test 4 years ago was negative. I am also hypothyroid and taking synthroid. I have 4-8 week cycles.

The problem: My last cycle was 50 days. On day 33and day 42 I passed a quarter-sized piece of white stringy looking, branch-like tissue. I also passed tiny pieces of white tissue (like confetti) on and of for the last two weeks of my cycle. I've been evaluated by a urologist b/c I also had pain in my left kidney area, but after extensive testing my kidneys were pronounced normal.

I started spotting five days before my period began (dark brown mucous, along with pink watery stains on tp). No cramps or PMS symptoms (very unusual for me).

My actual period lasted 7 day and was normal in heaviness and clots, but unusual b/c there was no pain. I usually have almost debilitating pain for 1-2 days.

I spotted for 5 days after my period (brown sticky mucous and bright red spots) and then one day three days later.

I started having period-like cramps and low back pain (and a few sharp ones that radiate to my leg) on day 8 of my cycle and they have continued (I am now on day 21) almost ever day.

I went to see a new gyno last week who didn't want to do a pap b/c I had one only 6 months ago and she felt that this was normal for cysts.

Has anyone else done this? Could it be a cyst? Should I be concerned since the doc wasn't? Why would I have period cramps a week + after my period and not during it?
