Default Miscarriage #3 and Metformin....

Iam just alittle ticked off right now. I just had my third miscarriage on thursday ending in a d&c. I was almost 10 weeks the other two happened before 6 weeks and ended on their own. I was sent to a RE when I had my second miscarriage they did lots of testing and the only thing they found was that my insulin was still elevated. They uped my metformin to 1500 mg time released and decided to try me on clomid to help produced better quality eggs. I have always had periods and never much trouble getting pregnant. I responded well to the clomid produced 5 follicles and got pregnant the first round couldnt believe it. I went in to have my levels checked 14 days past ovulation they were 113 and at 17 they were 485 progesterone over 45. They said everything looked great had me come in for an ultrasound two weeks later. So i went in at 61/2 weeks had it done everything looked great saw heartbeat was at 123. The Re tried to reassure me that my miscarriage rate was in the single digits and they thought everything would be fine and to send them a pic of the baby, they then turned me back over to my ob. They did drop my met to 5oo mg on that appt. I made an appt with my ob for 3 weeks later went in at almost 10 weeks and when they did an ultrasound there was nothing there just an empty sac like the baby just dissolved. I asked about lowering the met he said he didnt think it caused anything. I have done tons of research and found similar situations when met is dropped or stopped. I just find it hard to believe it wasnt the cause maybe.

Iam just really kinda pissed off now. I was sad depressed still am but now mad to. I was just wondering what your thoughts were on my situation and if anyone has ever went through this or stayed on met? My ob had told me before he was going to keep me on first 12 weeks and the Re was saying he likes people off by 10 weeks. Iam scared now to try and get pregnant again this time was worse with the d&c and stuff and I will be going back to the Re and I was some answers tired of hearing it just happens cause I dont think it does three times. Sorry I was ranting I just want some help

I don't have PCOS but will be starting metformin two weeks before the cycle I ttc. I have had five miscarriages. (However, I have two healthy babies mixed in there too). Insulin resistance was diagnosed after the 4th miscarriage. The plan was for me to go on a low carb/ high protien diet. I did this perfectly and felt the best I've ever felt. The problem is that I get sick when I get pg. Not normal morning sickness, but hyperemesis. Even with zofran and phenergan, I still couldn't stay on the diet. I've done some reading and diet and exercise can actually be as good as metformin in some cases - but you have to be able to do it!

I had to sign a waiver about using metformin for an off-label use. Though the metformin can help with the uternine lining, I don't think that's my issue since many of my losses are late enough that it's likely not a factor. The off-label use according to my RE is staying on it while pg. She wants me off of it once I make it through first trimester. I told her that I'll stay on it until I can do diet and exercise perfectly again. She wants me to start it two weeks before the cycle I start ttc again, so I'll do that. (I'm 7 weeks post d&c and still haven't had my first cycle yet. Plus, DH has surgery in two days since he might have testicular cancer so we have other issues than IR to deal with right now!)

Were you put on progesterone suppositories? Despite my progesterone not being proven low, both my OB and RE put me on progesterone suppositories as a just in case since levels can change so much. I've been on them 3 of 7 pgs, including the two that made it. Another thing I do is to take a baby aspirin a day. Part of this has to do with my clotting disorder, but I'm also told that it can help with auto-immune issues. (I don't have any proven but they're often hard to prove!) My dr did say that metformin won't help if my IR is caused by an auto-immune response though.

I have been tested for auto immune disorders and the blood clotting disorder and both were negative. I have read where you can test neg for blood clotting and then positive so I have thought about that to. My ob said it could take me two months to start my period. I would be just grateful to have one healthy pregnancy right now. Iam guess all I can do is wait and see what my RE has to say. My Re is not to big on progesterone I asked him and made him check mine he told me that if its already low that the pregnancy is usually doomed and it could just prolong it. I dont see why taking it before would hurt though. The did stop checking my levels after they thought everything looked good. They had mentioned trying baby aspirin but we didnt which I will probably mention. I did have an issue with bleeding alot when I had my d&c and they had to give me something to stop bleeding. Anyway thanks for responding Iam just very frusterated

I am so sorry about your losses. I, too, lost a baby and had to do a D&C (at 12 weeks). At 8 weeks I found out I was pregnant, got the heartbeat/pictures/ect., and I went off Metformin at my doctor's request. I'm not sure if the Metformin had an impact or not. But I think I will stay on it until the 2nd trimester next time. I also feel confused over the different doctor's giving different advice. Sometimes you just have to make up your own mind and do what you think is right for you.

Those of you that did miscarry did u start bleeding cramping etc? with my two others I did this one was different I had no idea no bleeding anything just went in and it was gone which makes me even more scared.

I had bleeding with three of the five miscarriages beforehand. With another one, I had spotting and then an ultrasound showed no heartbeat, it was a missed miscarriage. With my most recent one, a missed miscarriage as well, I had absolutely no bleeding nor any signs. I had weekly ultrasounds due to my loss history. There was a good heartbeat and growth at both 6 and 7 weeks but no heartbeat at 8 weeks. I was too sick to function even after the baby had died with my hyperemesis meds so I'm guessing my hcg numbers were still very high. I did have the fetus tested with the last loss and it showed a perfectly healthy baby girl.

BOTH my RE and OB are very pro-progesterone for my case. My OB did mention it was controversial, since it can prolong an already doomed pregnancy. However, that's a small price to may if your body may need it. I'd be requesting it after three losses if I were you, even if your progesterone tests fine. Also, make sure it's the vaginal suppositories. They absorb much better than the oral pills. Both my RE and OB only prescribe the suppositories due to this reason. (Well, except for the injections, but that's for women with proven low progesterone).

*My RE does weekly ultrasound for recurrent pregnancy loss cases 5 through 10 weeks, at which point I would have been switched to my OB if everything had looked good.

There is more than autoimmune and clotting disorders that you can be tested for too. My husband and I both had karyotypes done to look at our chromosomes. My husband had a sperm analysis as well. I had an hsg. I'm not sure how many things I ended up tested for, but it was somewhere around 20 different tests! (And some twice, like thyroid since that can fluxuate).
