Yes, you need to start the cream right away. If you're concerned about being pregnant then take a HPT and find out. Yes, the cream can cause a m/c if you are pregnant and stop using it - but I'm not sure how often this occurrs, if at all. You said you are extremely irregular so my suggestion is to use the cream for 2 wks on and 2 wks off until AF shows. After you are able to get your cycle down to 30-40 days start using the cream after O has been confirmed (either chart BBTs or OPK). If you know you have a problem with cysts then yes, follow the 21 days on 7 days off for 3 mos and this should dissolve them. Then after you've done this, start the 2 wk on 2 wk off method after you have confirmed O. Since you're irregular the dissolving cysts method might be tricky so I would suggest using the cream as stated above until your cycle shortens and becomes more predictable - that is up to you, though.
My cycles have improved tremendously in 4.5 mos (180 days, to 76 days, and now 36 days!). It may take awhile so you have to be patient. I really don't know what to tell you about your concern of losing 3 mos of TTC - this is true, you will lose a few months to get your body straightened out. I guess if you wish to conceive it will be difficult to accomplish this with cystic ovaries so really the wait is worth it (if you're a patient person). I think charting BBT's is VERY important cause, you never know, you could start O'ing right away and you'll never know unless you chart. Everyone reacts differently.
