If you have mild pain- it probably isn't too much but you should still be seeing your doctor for regular ultrasounds to monitor the growth because usually if a cyst is casing pain it is larger than normal and NEEDS to be looked at. Sounds like your doctor is taking a back seat on your care in all honesty (sorry).
If pain is milder may just be a cyst that has hit a nerve, or one that is inflamed (kinda like a hemroid, lol) Give it ibuprofin every 6 hours for a few days and it may go away.
As for spotting, my doctor always wants to check me if I am spotting, and cramping together. One without the other mildly isn't a cause for him to get concerned.
If your cyst ruptures- I can tell you exactly what the pain would be like! You will literally hunch over like a old women and will not be able to stand strait, it will hurt to sit, and the only way it eases off is if you lie down and then it takes about 1/2 hour of not moving at all before it stops hurting so bad, but then you move again and boom! Go to the ER and demand they call your OBGYN!
Belive me you will KNOW if you have a burst cyst. If you get this pain along with back pain and pain going down your leg and it starts when you exercise/run/ ect but goes away still have it checked! And make sure they monitor your growth on the cyst!!!!

At your age (no offence) I would seriously look into having an ovary or both removed! It won't stop everything, but it will help. I just had one out and already have less pain then I did before- even with my other ovary growing cysts! And it will decrease your risk of ovain cancer (obviously) and breast cancer, not to mention if you have a full hystorectomy (I know that is scarry to most...but it will get you outta the woods on endo cancer as well that is common with this pcos!) Many women I talked with said it was better with the full hysto than with just ovary out. I was in hospital only 6 hours to get my ovary and tube out, I was carried in and I WALKED out!!!!
