Pcos? First let me say I have not been diagnosed with PCOS (yet). In fact, my OBGYN said I do not have it because I have regular periods (yes, I know that is not correct AT ALL). I do have insulin resistance, and my Endo said I may have PCOS as well. I had gone to my OBGYN for intermittent pain on my right side, which I thought might be a cyst. I never had one before, that I know of, but it was obvious to me that was the issue. They did an u/s and I did have a cyst. Only one and they said it was a normal ovulatory cyst. I have only felt it maybe a half dozen times, but I think it might still be there (the u/s was in dec)
Ok, now that that's out of the way... here's my story:
I was always a pretty skinny kid/teenager/young adult... weight fluctuated but always within normal weight limits for my height. My period was always normal 30-32 days. Always pretty heavy though, and lasted for 7 days. My first 2 kids were coceived the first month we tried. The last 2 took well over a year. Between my 3rd and my 4th I was quite heavy (220 lbs) and I had brown thick discharge alternating with watery pink discharge between my periods. My periods became 45 days apart and would last over 10 days. I lost a TON of weight, going down all the way to 120 and my periods became normal again. That's when I got pregnant with #4.
Off and on, I had odd symptoms... my throat felt tight, hair would fall out, etc. Had my thyroid tested a bazillion times, always normal. That is until last year, when I had an u/s that showed a mulitnodular goiter. Since my TSH levels were normal, it was diagnosed non-toxic multinodular goiter. Since, I had dry eye and I have a 2nd cousin who has grave's disease, I went on my own and had a TSI (antibody responsible for grave's) test done... and sure enough, it was positive. So, I was then diagnosed with Euthyroid Grave's Disease.
Since, I was under the care of an Endo for this problem, I thought I would mention to him about how I have since around the age of 21 gained crazy amounts of weight, just from eating carbs. I could eat 1200 cals a day, but if there were carbs at best I would just stay the same weight, but I easily gained 10lbs a week. I could, however, lose it pretty quickly, as long as I didn't have carbs. I was always hungry, could never get full. He took note, and ordered an OGTT with Insulin and C-Peptide, Testosterone, and A1C. As you can see below, my insulin and c-peptide are quite elevated (btw-- is this mild, moderate or severe insulin resistance????). He placed me on Metformin ER 500mg. I am not sure how accurate the Testosterone test was, because I ended up getting my period while getting my labs done (@#$@!!!!) and the first week of your period is when you have the lowest testosterone levels... so I wionder had it been taken at a different time in my cycle if it would have been elevated (any thoughts???)
Testosterone Free 3.1 REF (0.1-6.4)
% Free Testosterone 1.54 (.50-2.0)
Testosterone Total 20 (2-45)
C-Peptide (Ref - .8-3.1)
1/2 hour 6.6
1 hour 6.3
2 hour 6.5
3 hour 2.8
Insulin (Ref. <17)
1/2 hour 39
1 hour 35
2 hour 31
3 hour 10
I won't list the other tests I had done, because they were normal.. including trigylcerides, a1c, cholesterol, etc. My chol. however did jump from 130 to 168 in just a few months.
I started the Metformin only a week ago, and I don't have many of the problems a lot of people have (GI -- just for the first day). For the most part it has made me feel a LOT better. However, my period is a couple days away and I have stabbing pain in both ovaries, wayyyyyyy worse than I ever did before. And like I said, I have only felt that pain intermittently over the past few months, never before that). Could it be that maybe the Met is making the cyst burst? It just seems coincidental that it got 100x worse after taking the Met.
I should also mention that my periods are normal, and I am PRETTY sure I ovulate. They are about 29-30 and the majority of times have discharge consistent with ovulation and sometimes feel a twinge around the same time.
So, what do you guys think... PCOS?
