1) Yes, they are pills.
2) I take 2 per day (which amounts to 1000 mg), but the bottle says that you may take up to 12!! So I suppose it's a personal choice, and it depends upon your symptoms and what you feel comfortable doing. If you do choose to take more, you'd probably want to start out small, and increase your dosages gradually.
3) Since I'm not ttc, I have absolutely no idea if this can be combined with fertility drugs. As far as met, I am on met (1000 mg per day), and I have had no problems taking both.
4) Side effects or precautions.... hmmm. Once again, I suppose this would depend upon how much you take. If you were to take a high dosage of this herbal formula, plus met, your blood sugars may or may not go too far down. You'd have to regulate your own response to the herb (as with all herbs or meds). I have been on this for over a month, and have noticed no adverse reactions, but I am on a small dose of both this *and* met. Plus, everyone responds to things in a different way, as we are all different ages, different weights, with different constitutions and different levels of severity in regards to our PCOS symptoms.
