I take it every 4-6 weeks or so for my asthma, including when I was pregnant with my son. My doctor told me the same - small risk of cleft lip, but the benefits vastly outweigh the risk. It's one of those cases where it's a risk/benefit evaluation. If there's more benefit than risk, they keep you on it. Totally a case-by-case thing, though.
Whenever I have drug concerns, I talk to my pharmacist. They know a LOT more about medications and side effects than doctors do. I was prescribed pred yesterday, with the doctor knowing there is a risk of pregnancy, so there you go.
ETA: I should add that I usually take a 5 day course, but it is much higher doses than people who are on long-term therapy. I believe people on it long term are usually in the 10-20mg/day range. I take 50mg/day for five days, sometimes up to 8.
