Question Question about RPL testing

Hi ladies,

I had 2 m/c's in a 5 month period so I recently went to the RE for testing. He has been very thorough and has run the following tests...

-Multiple transvaginal u/s's, an HSG, a saline sonogram, and an upcoming endometrial biopsy
-A full hormone workup on day 3 of my cycle
-A fasting insulin and blood sugar tests
-A SA for DH
-Thyroid tests
-Karyotyping for both DH and I
-Tests for HIV and STD's
-Immunity to Rubella and Varicella (and I think some other stuff)
-Some general blood tests to look at cholesterol, etc.

Should I also ask him to test for immunity-related disorders? If so, which ones? What tests would show if I have a blood clotting disorder? Can you think of any additional tests that I should ask him to do? I'm just trying to get a complete look at our situation so we know exactly what we're dealing with.

Hi Shari. First of all, I'd like to say how sorry I am for your losses.

I just went through RPL testing myself. I had a hysteroscopy, an ovarian reserve test panel (because I'm 32) which thankfully came back very good - the RE said I have a reserve of someone in their mid-20s - Day 3 full blood work-up, a "Sperm DNA Integrity Test" for DH because sometimes his morphology is a bit low, and the clotting/thrombophilia work-up done. Everything came back fine EXCEPT some clotting problems were detected. I did NOT test + for any hereditary thrombophilias (such as Factor V Leiden or MTHFR) but for "acquired" ones. My anticardiolipin antibodies were high, my lupus anticoagulant antibodies were a low +, and there were a few other specific ones I tested positive for that I can't remember off the top of my head.

These are the specific immunology tests they offer/recommend:

antiphospholipid antibody (APA) panel, (NOTE: This is what I have)
antinuclear antibody (ANA) panel,
antithyroid antibody panel,
embryotoxicity assay,
reproductive immunophenotype,
natural killer (NK) cell assay,
immunoglobulin panel,
anti-IgA,
coagulation assays,
lupus-like anticoagulant, (I tested with a low positive for this as well)
activated partial thromboplastin time,
protime,
homocystinemia,
antithrombin III,
factor V von Leiden
inhibin B (this is the ovarian reserve test)

Hi, I've gone through RPL testing recently, but I only had blood tests. And my testing was not as thorough as yours or Meghans. Would you mind telling me what hormones you were tested for the CD3 complete hormone test? Thanks and good luck to you.

After my m/c 's, I only had the hysteroscopy (they discovered a septum in my uterus), so we didn't need to look any further to find the cause. I can't offer any advice on the other tests, as I haven't had them.

I just wanted to send you {{HUGS}} and a prayer that your doctor is able to find and correct the cause of your m/c 's.

Nani - as far as the hormones go, I think that when your ovarian reserve levels are tested, it needs to be done on CD3, 4, or 5. I'm sure FSH is one of the hormones that is tested in this process. I am sorry, i don't know much more about it...

Thanks to all of you for your responses. I will see my RE in a little over a week, and I will ask about ANA, APA, and some of the other things you mentioned. It just seems like so many tests, and I'm not sure which ones my insurance will cover (if any).

Nani--I don't remember exactly which hormones they tested for on Day 3 (and I'm still waiting on results, so then I'll know for sure). I think it included FSH, LH, Prolactin, testosterone, estrogens, DHEAs, sex hormone binding globulin, thyroid hormones, insulin and glucose. There may be other things, but I don't remember. They took so many viles of blood! I'll let you know once I get all my results back and can see it all on paper.
