After reading your story, I was very inspired that you realized health/personal as well as professional accomplishments.
I am pursuing my dreams and suffered burnout after trying to advance my personal, professional and service efforts. I want to be the best, yet I was doing so much that all of my efforts were mediocre.
I know that health/self is my #1 priority.
Did you have to pare down your responsibilities/obligations during that first year that you committed yourself to healthy eating and exercise? Or, did you prioritize and pursue multiple personal/professional goals simultaneously?
I know what I need to do and how I need to do it, I just feel stuck because I tend to get involved in EVERYTHING that I'm interested in.

I pared WAY down. Waaaaaaay, down, and rebuilt my responsibilities from the ground up, so that I could identify what habits needed to be the foundation of my 'routine' once the other responsibilities were re-introduced.
My 'professional' life was put into a holding pattern for an entire year.
