1.What is your training in infertility? Are you board certified as a reproductive endocrinologist?

2. What hospitals are you associated with?

3. What insurance plans do you accept? Do you have payment plans?

4. Does your fee include everything? Drugs? Classes on injections? Anesthesia?

5. What hours are you open? Can you accommodate my schedule?

6. What are your pregnancy rates for IVF? GIFT? ZIFT? IUI?

7. Is your lab set up to do Intracytoplasmic Sperm Injections (ICSI)?

8. Can you do Pre-implantation Genetic Diagnosis (PGD)?

9. Do you work with any support groups in infertility? Psychologist? Social worker?

10. Do you provide prenatal care? If not, at what point will I be released to seek prenatal care?
