Severe endometriosis new indication for fertility preservation

The first published report of fertility preservation in a woman with severe endometriosis and low ovarian reserve.
MedWire News: Young women due to undergo surgery for endometriosis should be counseled with a view to ovarian cryopreservation, says a Canadian team.
Their recommendation is based on a case report published in the journal Fertility and Sterility describing a nulliparous 25-year-old woman with severe, symptomatic endometriosis and low antral follicular count.
The woman had already undergone a right ovarian cystectomy that was followed by oophrectomy for uncontrolled bleeding. Her symptoms were only partially relieved by hormone therapy and she occasionally required morphine analgesia.
Although the left ovary appeared normal on ultrasound, the antral follicle count was just 3, indicating diminished ovarian reserve. As she had no male partner she was counseled about the possibility of oocyte cryopreservation before further treatment.
The patient subsequently underwent three cycles of ovarian stimulation and oocyte aspiration; a total of 21 mature oocytes were vitrified and cryopreserved.
"We recommend evaluation of ovarian reserve before treating women with severe endometriosis and particularly those with large endometriomas," write Shai Elizur and team from McGill University Health Center in Montreal, Quebec.
"In addition, fertility preservation should be part of the preoperative counseling in young women with severe endometriosis."
