I've been in your shoes (90 pounds to lose), and even without knowing how long and hard you're working out, I think your trainer is leading you down the wrong path, in terms of expectations.

Where Did All The Fat Go by Dr Rob Huizenga. He is the doctor who created the weight loss program followed by all contestants on all seasons of The Biggest Loser. The book follows the weekly workouts/diet/weight loss of the season 3 participants who were working out at home - and not at the ranch.
Starting week 1, they're walking/running 4 to 5 miles every morning, 6 days a week for an hour, and then at night, they're working out another 60-90 minutes. In other words, they're working out 2 to 2.5 hours every day...6 days a week. Their calories are in the 1250-1500 range.
On average, the women are losing about 1% of their body weight each week - not '15 pounds a month, every month' - and I don't recall ANY back-to-back WEEKS where women lost 4 pounds (of bodyfat) per week. While pcos isn't mentioned in the book, it's entirely possible that one or two (maybe more) of the women in the book have pcos, but not 'all' of them.
Another thing that's interesting in the book is that Dr H points out that in the first few weeks, some people might show a 'weight' gain on the scale due to water retention. That volume of exercise causes your muscles to swell. But body fat loss is still happening.
In the current pcos bootcamp at Fit Cysters, we're following the 2-a-day protocol in Dr H's book. Those following the closest, are losing at the rate in the book.
Unfortunately, many trainers don't REALLY understand about losing 'fat', and many exercisers underestimate just what's involved in even losing a 'pound' of body fat (with or without pcos).
Get the book. It might help you find ways to tweak your current program. You're clearly willing to put in the time, the book might help you get more 'bang for your buck', and will give you a better baseline to compare your results to.

I hear you...and if I were you, I'd TOTALLY tune out people saying they're losing 15 pounds a month...It simply isn't 'reality'.
Even playing field with MAX effort (ie 2 to 2.5 hours a day of intense exercise and calories in the 1500 range) will return a loss of about 1% of body weight - not 2 to 4 pounds of body fat loss per week. But that's ok. Even at 1%...you'll still get there at a rate that will seem lightening fast.
By the third week in this current PCOS bootcamp, most of us were a size smaller, and going into the 3rd week, we had decided to not to buy any more clothes until the end of camp, because we're shrinking so fast.
get the book, it's really a great read!
