Hi everybody! I'm new to the boards. I was diagnosed w/PCOS and IR in January and started on a low/good carb diet w/exercise in January, as well. I just saw my endo on Friday and now that she has my labs back, she gave me a prescription for Metformin. I've read all about the side effects and I'm really nervous to start this medication. I've always had a bad stomach, so I'm kind of used to diarreah. I'm just terrified of throwing up. Got some weird phobia about that one. Anyway, I'm starting the Met tonight w/dinner (as per endo). She's starting me on 500mg. She told me if the side effects become unbearable, to stop it, give my body a rest for a week or so, and then start over on 1/2 the dosage. I see her again in three months, at which time, I assume she'll up my dosage to 1000mg.

I can certainly understand your nervousness. I was nervous too!
What I think you'll find, though, is that the vast majority of people never throw up. From my experience, the big thing is diahrea and lots of tummy rumbling.
When I hit my full dosage I had some nausea and food intolerances so it was hard to stick to low carb. For about a month I ate carbs if I wanted to, just limited their portions. The thought of protein turned me off so I didn't eat a lot of meat during that month. I never gained any weight, though!
Eventhough I was nauseated some of the time, I never once thought I was going to throw up. It was just a mild queasy sensation from time to time. Taking the pills with food really really helped (I think the pills just irriated my stomach lining much like aspirin), so that's my recommendation. But truthfully, I don't even think that nausea is all that common, either.
Since you're really nervous, how about starting out really slow? Take that 500mg pill and cut it in half. Take half for a few days, then work your way up to a whole pill. Most people don't have a problem with the 500mg pill, but it certainly won't hurt to take 1/2 of it at first.
Chances are you'll be on metformin for the long enough that it won't matter if it takes you 1 month to get to full dosage, or 2 months or 3 months or however long.
