Current trends in the treatment of polycystic ovary syndrome with desire for children.

Polycystic ovary syndrome (PCOS), one of the most frequent endocrine diseases, affects approximately 5%-10% of women of childbearing age and constitutes the most common cause of female sterility regardless of the need or not for treatment, a change in lifestyle is essential for the treatment to work and ovulation to be restored.

Obesity is the principal reason for modifying lifestyle since its reduction improves ovulation and the capacity for pregnancy and lowers the risk of miscarriage and later complications that may occur during pregnancy (gestational diabetes, pre-eclampsia, etc).

When lifestyle modification is not sufficient, the first step in ovulation induction is clomiphene citrate.

The second-step recommendation is either exogenous gonadotrophins or laparoscopic ovarian surgery.

Recommended third-line treatment is in vitro fertilization.

Metformin use in PCOS should be restricted to women with glucose intolerance.

Good article Kat - though it's not what a lot of women will want to hear. It makes me happy that I'm making those lifestyle changes now so I won't have to struggle through them when I've finally got insurance and can see a doctor (assuming my body doesn't ovulate on it's own and I miraculously get pregnant without further meds).

It's interesting that the article doesn't recommend the use of Metformin to help with Ovulation though - I know that there are other studies that have shown it helps when used in conjunction with the Clomid. Of course that doesn't really matter for me since my last set of labs showed me as officially IR and until that's under control I'll remain on the Met (not taking chances with how prevalent diabetes is in my family).
