Hey There,
It is very confusing isn't it. Well, just so you know, I was told that I have "textbook ovaries" and there aren't any cysts to be found. Some doctors don't acknowlege this, but you don't have to have cysts to have PCOS. My experience was that my OBGYN woudl only diagnose me with PCOS if I had cysts, but my endo actually did all of the tests and concluded that I am insulin resistent and I do have PCOS. The bottom line is that not all doctors know enough about PCOS to realize you don't have to have cysts to have PCOS. I haver "perfect ovaries" but I have PCOS. You just have to find a doctor that really knows about PCOS.
"A PCOS diagnosis cannot be made entirely on the basis of the ultrasound alone since not all women with PCOS have cysts.
This may seem contradictory. Why call it polycystic ovary syndrome when not all women have cysts? It is because PCOS was identified (originally as Stein-Leventhal Syndrome) many years ago based on the presence of cysts in the first known patients. Conversely, not all women with cysts have PCOS. The presence of cystic ovaries does not always mean that you have the syndrome.
If PCOS is suspected, your physician may recommend an extensive check of your endocrine system."

I hope that answers some of your questions- just know, where in the same boat. It actually took me over 2 years to find a doctor who would ackowlege that something was wrong with me. Hang in there and just keep pushing and be strong!
