Does anyone know if it is covered by insurance once its prescribed?
If not how much does it cost?

All insurances are different, but mind (BCBS) does not cover it.

I went through drugstore.com and they had a special where you got $25 off one tube or $50 off two tubes. they even contacted my doctor for me. I think it was like $70 for two tubes shipped. the tubes are small, but you don't use very much, since your face isn't a big area.
