It can be any of the three just as Stacie pointed out, however I did find that my PCP was reluctant to prescribe it to me on the same dose that my reproductive endocrinologist does (which is twice what my PCP prescribed). My PCP just did not feel comfortable prescribing Metformin 1000 mg twice a day, which is the recommended dose for PCOS patients.
