Thanks for the replies! I've called the insurance company and they said I need the letter from the doctor that includes the 5 year history,height, weight and BMI. The doctor's office I called said they worked with United before, so that's a plus.
I'm going to see Dr. Lauter in Bellevue... anyone WA cysters have their surgery or are seeing him?

And asking for prayers. I just found out that my grandpa's lung biopsy came back as a bad form of cancer. Thats all we know right now. So any thoughts and prayers we can get for him would be great. I'm not asking God to keep him here with us if it's his time to go. All I am asking Him for is that Papa enjoy his time left on earth, whether it be five months, or fifty years.
